Question potting soil

so ive been trying to mix my own potting soil and ive been having some problems. probably mostly with the ratio. can anybody give me an idea of a good ratio to use and maybe if i would need anything else? here is a list of the ingredients i have at my disposal:

peat moss
coarse vermiculite
2 years aged horse manure composted with 10% wood chips *i have lots of this crap
mushroom compost
worm castings
hydrated lime
dolomite lime

any ideas???

Here is a link to a recipe. But I think some of the ingredients are different than what you listed.
