Citrus not producing

Approximately 4 years ago I planted a Meyer Lemon tree as well as an orange tree (forgot the name). Every year since then I get one single delicious orange from my orange tree and 1 single huge lemon from my lemon tree. That's it! The foilage appears to be nice and healthy and both receive the required 6-8 hours of full sun daily. Any advice or suggestions as to why they are not producing more?

Yes............I use a fertilizer specifically for fruit and nut trees but I have a horrible fear of overfertilizing so I go very lightly. My Dad is famous for killing everything with fertilizer so it's made me somewhat gun-shy.

I thought of scraping the surface beneath and adding organic cow and chicken compost. The reason for scraping was to prevent the soil from building up to high around the trunks if I were to continue adding amended soil to the existing.
